Fill the form no.2 in duplicate. Also fill the Form Nos.1 & 27A and submit to us along with your FDA licence copy, list of products for which manufacturing licence is given and list of products which you want to get irradiated at our facility. After checking, these Forms 1,2 & 27A and list of Technical Staff will be returned to you for due submission to FDA.

We shall provide to you our consent in duly filled up Forms No.3,4, & D alongwith a copy of our FDA licence which you can submit to the FDA.

Pay the Licence Fee (Which is for 5 years) alongwith all the above documents to the FDA office at Thane. The address is - Vardan Building, Road No 16, Wagle Industrial Estate-Thane West, Thane - 400604, Near Old Passport Office.
